A fully clothed little girl engaged in an explicitly adult activity, which implies other uncharacteristically adult activities by association, who was photographed by her own mother -- not a woman of strong moral character, if she's not just letting her kid smoke but is taking photographs of it -- and the book from whence the image comes being one that has landed people in hot water with the cops vis a vis kiddie porn investigations for having had it on their computers?
